Subject: J-51 status

Does anyone know which city agency administers the J-51 program? I would like to verify the J-51 status of an apartment building and don't know who to contact. Also, do any documents (such as a notice of decontrol) have to be filed with the DHCR when the tax benefits expire? Any information on the J-51 program would be appreciated.
