Lease Succession Questions

Hello,
I live in a rent stabilized apartment in NYC. I want to take over the lease from my sister, who is the leaseholder (i.e."tenant"). I have lived (as an "occupant") in the apt. for 3 years and have paid the rent during that time.
Questions:
What forms do I need to file in order to take over the lease? Do they come from the DHCR?
How do I need to notify the landlord?
What proof will I need to show that I have lived there for the mandatory 2 years?
My succession rent increase should be less than a vacancy lease, correct?
Do I have any recourse if the landlord was late sending the renewal forms (less than the mandated 90days)? I assume no.
