Put repair requests in writing and send to your landlady.

Since you want to move out, your best tell her professionally (avoid accusations or name calling) that since you have serious issues with the apartment you are going to look for housing elsewhere. Tell her you don't want to inconvenience her with more repair requests and ask if she would agree to end your lease by mutual agreement. Very likely she'll agree.
